Pharmacist Consent Notice

To be shown to, and accepted by, the pharmacist. Do not accept it on the pharmacist's behalf. Record the time and method of consent. The pharmacy and DavaSetu should each keep a copy.

4.1 Why you are seeing this

the pharmacy (the "Pharmacy") uses DavaSetu, a software platform operated by Youhexa Technologies Pvt. Ltd., to run its public website. The Pharmacy wants to show who its registered pharmacist is. To do that, it needs your permission to publish some of your details.

4.2 What will be published publicly

Your full name. Your pharmacist registration number (and the council/state of registration, if entered). Your photograph. That you are the pharmacist at the pharmacy, the pharmacy's registered premises.

This information will be visible to anyone on the internet at the Pharmacy's website (for example yourpharmacy.davasetu.co.in) and through its QR code. Search engines may index it.

4.3 What will NOT be published

Your registration certificate or other documents, home address, personal phone number, personal email, date of birth or ID numbers. These are not published. Any documents you upload are kept private and used only for the Pharmacy's records and DavaSetu's internal review.

4.4 Purpose

Your details are used only to show the public who the pharmacist at this Pharmacy is, as part of the Pharmacy's own public information website. They will not be used for advertising, sold, or used for any other purpose without your separate consent.

4.5 Your choices and rights

Consent is voluntary. You may refuse. If you refuse, your details will not be published.

You can withdraw consent at any time. To do so, tell the Pharmacy, or email support@davasetu.co.in. Your details will then be removed from the public website within 48 hours of your withdrawal reaching the Pharmacy or DavaSetu. Withdrawal does not affect publication that happened before it.

Withdrawal may affect the Pharmacy, not you. If the Pharmacy is required to show pharmacist details, withdrawing consent may mean it has to appoint or display another pharmacist. That is a matter between you and the Pharmacy.

When you stop working at the Pharmacy, the Pharmacy should remove your details. You may also ask us directly.

4.6 Consent

I have read this notice. I agree that the pharmacy may publish my name, pharmacist registration number and photograph on its DavaSetu website for the purpose described above.
